In April 2007 I had a laparoscopy which revealed stage 3 endo – on the outside of my ovaries and all over my bladder which resulted in it being ‘glued’ to my uterus. The endo was removed. I’d like to know what the chances are that it has returned as I never really had, to me that is, any obvious symptoms of Endo as I’d always had painful periods and often discomfort during sex – symptoms which I currently have. On what grounds does an FS decide to do a lap to check for endo, and what are the chances that my endo has returned?

The likelihood of stage 3 endometriosis returning is approximately 75% over a 5 year period. ( Probably sooner the worse the disease) Whether or not to repeat surgery is usually based upon a high index of suspicion, previous history and symptoms.

I have stage 4 endo, my husband has a low sperm count. I’ve gone for 2 laps in 1 year. Removed endo stage 4 in 2007. Gynie appointment in 2008 showed no endo? My question is – should I go for another Laparoscopy, followed by IVF, or should we continue to try and have a baby on our own?

Based on the history that you have provided in the question, I think that a laparoscopy followed by IVF/ICSI is indicated.

Stage 1 and 2 endometriosis qualifies as mild to minimal. In cases where surgical excision has been complete, the recurrence rate is about 20% over a 5 year period. Stage 3 and 4 is severe and the recurrence rate is 75% over a 5 year period. It would therefore be wise to repeat the surgery as soon as the symptoms re-appear in the case of previous stage 3-4 endometriosis.
